Home Care Coordinator - Intensive Case Manager

Promotes and maintains the health of individuals in the LIFE community through teaching, counseling, and appropriate preventive and rehabilitative services. Assists with the assessment of health needs and contributes to the plan for care for individuals and families. Responsible for oversight of home, hospital and nursing home care. Completes audits at the nursing home, home care agency and participant’s home in accordance with the Contract Oversight and Quality of Contracted Services policy and procedure. In addition, coordinates all home care services including all nursing home care under the supervision of the Director of Health Services. 

SHIFT: 1st shift Full time 40 hours/week

Social Worker - BSW

Promotes and maintains the health of individuals in the LIFE community through casework management services that contribute to the success of the plan of care for participants and families. Coordinates the recruitment, orientation and supervision of LIFE volunteers. Assists, with the intake and enrollment process and with the initial and ongoing Medicaid eligibility process as directed by the MSW. Responsible for the administration and coordination of home care services.

SHIFT: 8:00am - 12:00pm Part time 20 hours/week
