Adult Day Health Center

The LIFE Center is a homelike and safe environment that includes areas for medical treatment and rehabilitation, personal care, dining and recreation. Participants in the LIFE program may attend the LIFE Center, open Monday through Friday (excluding holidays), between 1 and 5 days a week, depending upon their need. Transportation will be provided, via specially equipped vans, to bring participants to and from the LIFE Center.

The services that follow are all provided at the LIFE Adult Day Health Center:

  • Occupational Therapy
  • Speech Therapy
  • Recreational Therapy and Activities
  • Nutrition Counseling
  • Meals and Snacks
  • Personal Care (Bathing and Grooming)
  • Transportation to and from the LIFE Center

Once a person enrolls in Albright LIFE, all services, with the exception of emergency care, must be provided and/or coordinated through Albright LIFE. The participant or family must pay for any unauthorized services from outside the Albright LIFE network. 

Physician & Nursing Care

At Albright LIFE, our team of experienced and caring health care professionals focus on the participant and his or her needs.  As part of the Albright LIFE enrollment process, a team of nurses, an elder care doctor, social workers, rehabilitation specialists and other health care professionals assess the participants needs and create an individualized care plan.  They get to know each participant, his or her living situation and what he or she does and does not like.  Because LIFE is a provider sponsored health plan, the LIFE health care team works with each participant to make decisions about his or her health care.

All primary medical care is provided at the LIFE Center's Medical Clinic with same day appointments and full medication management. LIFE nursing staff is also on call 24 hours a day/7 days a week.

Inpatient Services

The following services are covered for participants in the LIFE program:

  • Audiology and Hearing Aids
  • Dentistry
  • Optometry and Eyeglasses
  • Foot Care
  • Mental Health / Substance Abuse
  • Physician Specialties
  • Medical Equipment and Supplies
  • Radiology Services
  • Prescription Medications
  • Preventative Health Services
  • Non-Emergency Transportation
  • Lab Tests and Procedures
  • Outpatient Surgery
  • Emergency Room Care

Caregiver Support

Our Social Workers and Chaplain provide support and counseling to both caregivers and participants. They are available for information sessions and counseling and can provide information about government programs and community services. Support groups facilitated by our Social Workers are offered to caregivers so they can talk with others in similar circumstances.

Transportation

Transportation is provided via specially equipped vans to bring participants to and from the LIFE Center and medical appointments. 

Nutrition Care

Good nutrition is an important part of our participants’ health and well-being. Albright LIFE’s dietician understands the specialized needs of seniors and meets regularly with participants to provide nutritional counseling and dietary planning.   Meals are provided at the LIFE Center according to each participant’s personal dietary needs. This may include a light breakfast, lunch, snacks and/or a packed meal that can be sent home if needed.   

Medicine Management

Albright LIFE participants receive all medically necessary prescription medicines at no cost to them-there are no deductibles or co-pays.  Participants are responsible for any over-the-counter (non-prescription) drugs.  Albright LIFE's nurses manage, order and distribute all participants' medicines. 

In-Home Care

Albright LIFE is dedicated to helping seniors live safely in the community. Thus, LIFE participants may receive some in-home care services, which may include the following. 

  • Nursing services
  • Home-delivered meals
  • Physical, speech, and/or occupational therapies
  • Medical services
  • Social services
  • Personal care services (help with dressing and bathing ect....)
  • Homemaking/chore services (groceries, light meal preparation, etc...)
